Septic to sewer conversion ( clean out pipe ) is this to code ?

My concerns are that a clog occurs accessibility to the (cleanout pipe) is obstructed, and the face of the pipe opening is towards the foundation and below grade. So basically when 4 inches of gravel and 2 inches of asphalt is applied because it is on an angle either the pipe will be buried under the asphalt or half of the top cover will be buried leaving the other half exposed. The attached pictures will show that the (clean-out pipe ) due to the “ single city 45” angle pipe “obstructing plumbing machine access and a protective sewer cleanout box cannot be installed on the driveway. Leaving it exposed to damage. The installed pipe position further impedes waterproofing eventually leading to an erosion area around the pipe and foundation. My understanding is that nowadays a 2-way cleanout is the preferred pipe method installed to grade/flush with pavement or WYE pipe with a 1/8th bend combination.

I would appreciate any advice or comments about this (cleanout ) and the connection

Some idiots need to come back and do it right.
It is no good the way it is. A cleanout tee (also called a test tee + plug on its back with a big concrete utility box over it might be the best.
