Replace Cast Iron Clean-Out

I have an old cast iron clean out that I would like to replace. It serves one bathroom and vanity. I was wondering if I will need to support the upper portion of the remaining pipe both during and after replacement. Is the PVC section going, along with the new PVC clean-out, going to be strong enough to support the weight of the cast iron pipe above? Is there a possibility of things shifting after replacement? I would say that there is about 5 feet of cast iron pipe that will be remains, the majority after a 90 degree elbow about two fee t above the clean-out. We are talking 4" pipes. Thank you very much for any and all advice.

Yea around here they are called midstory supports good practice to keep the weight off pvc.
 
Why are you looking to swap the cast iron clean out for PVC?
