If it's a residential heater in a commercial setting, no bueno. Not built for the demand.
Also if that much grease is sitting on top, I can imagine what the burner looks like. With all that being said, with the new FVIR standards, need to make sure flue is clear and there is nothing obstructing airflow through screen ant the bottom. Probably nasty down there.
But is it a commercial grade WH?
Do you mean the actual thermostat is set above 120 degrees? Scalding hazard. How about a tempering valve install?
If the burner plate is covered with sediment, the tank cannot fully heat the water (may hear gurgling or pounding in the tank). The sediment will also anneal to the plate and even after flushing may not heat properly.
Is this just a service call or are you somehow obligated to it by a warranty?
2nd Thought...
Maybe defective heat trap nipple(s)...
