The inspector said this is DWV connection is illegal?

I had my town plumbing inspector out Tuesday to inspect my 2nd addition plumbing and he said this existing plumbing connection to the sewer pipe is illegal. Can anyone be nice enough to tell me why perhaps so I can correct it? He said my plumber would know why... My guess is because the Y connections are flat instead of angled like the sewer Y and that's the reason.

FYI: The closer 2" PVC is for the kitchen sink and the farther one from the sewer pipe is for the Washing Machine.

Lastly. I need to connect the 2nd floor addition DWV into the sewer pipe here so one idea was to just put a bushing screw on the end of this and a sweep into it with a cleanout at the end but before I can do that, I need to know how to correct this problem.

i see 5 violations , the 3'' should have stayed 4'' clean outs shall be the size of the line it serves

you have a 3'' co on a 4'' line.

2 violations on the 2'' cleanouts, the type installed are for a vertical installation, you need to use Wyes with a clean out plug

and last is see, hard to see, back in the back a unvented ptrap , and a hose stuck in the pipe with no air gap

Thanks for taking the time to answer my question. I can definitely see what you mean. I signed up for 1 year to say thanks.

So I'll probably cut the line pvc before the illegal wyes, then run new wyes with the angle serving the pipe that it is cleaning out ( not towards the sewer ) with an adapter and plug. I'll cap the illegal drain also.

So if it's alright I have a continuation question now that I know how to correct this situation, my next problem is adding a 3" pipe to that sewer connection. Obviously it's starting to get pretty long with 3 wyes sticking out of the sewer pipe. Would it be legal to connect the 2, 2" sink/dishwasher and washing machine lines into a single 3" line and then connect into the 4" sewer. Then my other new 3" connection can join as well and its basically sticking out the same as before. I'm going to illustrate what I mean with a photo and some photoshop. I'll post the pic in a few minutes. And of course, all pipe straps would abide by your previous recommendations.

Not shown on the diagram are cleanouts every 90 degree change of direction for the new line.

Ok, here is the dryfit layout I did this morning. Do you see any violations with it?

A tip: NEVER dry fit a layout. The pieces fit significantly differently when glued together and all your runs will end up short. In other words, dry fit layouts never fit properly when glued. You have done alot of work, take heed of what Frodo says and you should be good with your inspector.
