Sewer Clean out in odd spot.

In the house we are looking at I have found that there is a clean out in the formal living room.

I mean right there in one of the walls near the floor you can see a cap.

That cant be normal right? The house is on a slab and so there is no basement or crawl space.

Have any of you ever seen this before?

In most cases with the house on a slab the clean out is just outside were the line exits. Is this clean out you have on the main or a secondary line?
