I'm not a plumber, but would like to fix this myself, if possible. Hoping somebody can point me in the right direction. Toilet is low-profile. Apparently made in 1976. (stamped inside tank). Don't know the maker. Internal is marked Kohler but hard to make out the numbers. Internal tank width is about 5". Height of tallest internal component is about 10". I need to replace everything in the photo, if possible. Can you give me a clue?

You could probably buy a new toilet for what a complete rebuild would cost.

If you want to rebuild it, send all your pics to Chicago Faucet Shoppe. They have a email address designated to send pics. They should have the parts if they still are available.

You need a Kohler 1B1X conversion kit

I’m not saying those don’t work for some people but I’ve had bad luck with them. The toilets wouldn’t flush good after it was installed. Like it wasn’t getting enough water.

Thanks for the quick responses, and BlueSkyHigh's touching prayer!

Did a quick internet search and found the Kohler 1B1X conversion kit for about $90. Seems like it would make sense to try it before an expensive rebuild or buying a new toilet. My follow-up questions are... 1. Are you pretty confident this kit will fit my toilet's configuration? 2. Is the kit easy enough for a toilet-fix-newbie (relatively healthy old dude with average intelligence) to install?
