water freezing

ive been in my house now for 3 years. every winter when the temp falls below 28, some of the cold water supplies will freeze. the pex is insulated with pipe insulation and has 3-4" blown in on top of it. granted they are run down the exterior walls in the bath and kitchen but i have r19 in the walls and all the areas between the trusses are insulated and blocked off with additional solid soffits. the exterior of my house is block and the inside is insulated and covered with sheet rock. do the fittings usually cause a freezing problem? any ideas will be MOSTLY appreciated. thank you
 
It is possible there is an air void or something like that in your insulation. Obviously the cold is getting in somewhere. Does the freezing seem to be located to a single area?
