Help with Toilet Flange Levelness

I am remodeling my bathroom and have a question about the levelness of the existing toilet flange (picture attached). It is not level with the floor and is off by about 3/8" from back to front. In a perfect world this would be level and set flush with the finished floor. How bad of a problem is this and should I try correcting the problem which will require some serious work to fix due to access restrictions. :(
 
Does it move up and down at all?. Looks like it needs to be screwed to the floor.
use brass screws. Looks to be kind of high also. Are you putting down tile?
If the finished floor is going to be built up so the flange is almost flush to the floor then you maybe alright.
It's sealed to the toilet with wax and as long as the flange can't move and the toilet is set and doesn't rock then it should be okay
 
I still have to put down 5/8" plywood which the lowest edge of the flange will set on. I had planned to use wedges on the high side and screw down but was worried about the 3/8" difference in levelness from front to back.
 
I would install the plywood, cutting it to fit underneath the flange. Then, using brass screws as Mr. David mentioned, and using 2 or more holes on the front of the flange, see if you can pull it down somewhat. Be careful, and don't try to get it perfect, as plastic, especially older plastic, can break if you stress it too much. Once you get it a little more level, use those wedges to shim to the bottom of the flange.

When you set the toilet, make sure that you shim the toilet so that it does not rock at all. Many people just use grout to hold the toilet bowl steady, but I have found that this is a recipe for a cracked flange and a rotted floor. Get the closet bolts kinda snug, shim the toilet so that it doesn't rock, and tighten the bolts a little more. Then grout.
