Layout Advice Needed

Hey everyone. My 1975 galvanized pipes are starting to spring pinhole leaks and it's time to re-plumb. I'd like to try to do it myself, and replace everything with pex. I like the idea of having a manifold but I don't know if a manifold is always a good idea. I'll paste a copy of the layout of my house (not to scale) with a manifold based layout at the end of this message, but I have a few questions. The main question is based on the layout of my house, should I or should I not use a manifold, centrally located in my utility room? If so, then here are the rest of my questions:

1) From the manifold, should I run one output per room (which is how I've drawn the rough layout), or per fixture?
2) What diameters should I use for each section?
3) ...and if I stick to one output per room, then should the "trunk" for each room be a bigger diameter than each branch in that room? (As in should I use something like 1" for main supply, 3/4" for each "trunk" and 1/2" or 3/8" for each branch?)
4) The main water supply comes up out of my driveway, then to a T. One branch of the T goes into the house through the wall, and the other branch has an outside spigot. I can't recall this ever causing pressure drops but I'm wondering if that's really a good idea. Should I change this when I re-plumb?
