How can a toilet break like this? Any Ideas?

I just bought a new rental property. The toilet was draining slowly. Today I pulled the toilet and replaced it. All is well. I am attaching a picture of the bottom of the old toilet. The ceramic that would contact the wax ring is destroyed. Any ideas what could cause this? There were not any broken pieces in the old wax ring. Do you think someone installed it like this? Just curious.

A sewer cleaning cable may have come up into the toilet arm and broke the horn off.

I’ve done it several times

A closet auger can do it too.
