Hot water stem leaks

Hi. I am not a plumber, although I think I understand some basics of plumbing. Maybe I don't know as much as I think. Anyway, I could use some help.

I have an old shower system in my house with Kohler fixtures. I've replaced the hot water valve (stem) several times over the years. Now, water shoots out of it once I open the valve. It will even shoot out water when it is closed, but the cold water is opened. If both hot and cold are closed, there is no leakage of any kind.

I don't know what do. It is a new fixture, with gaskets and washers in new.condition.
 
Could be the stem packing leaking or if it's a Kohler valvet there is a washer and o-ring that go over the stem to prevent it from leaking. Kohler makes a few different styles of faucets without a picture it makes it hard to tell you exactly what needs to be done.
