Gurgling Kitchen and Island Sinks and Strange Plumbing?

There's always been some occasional gurgling from our kitchen and island sinks. Lately it's become constant, and the sinks are slow to drain. Though we built the house 20 years ago, I never thought much about how the sinks were plumbed...until now.

From what I've read and researched, the current island plumbing isn't close to being effective, let alone close to code?

Here's the current configuration...

The photo shows where the island sink drain pipe drops through the floor and meets the main drain pipe. The wye connector enters the main pipe on the side. Would this also contribute to the drain problem? Should the wye be oriented to enter the main pipe from the top? (There's not a lot of room to play with here.)

And while I'm at it.. should there be a cleanout on the pipe above, under the cabinet?
 
There seems to be more slope than meets the eye? Maybe I'm obsessing too much!

Yes...the closeup is the section of pipe going into the wye connector at the drainpipe. I agree, it looks flat but the level is pressed against the bottom, and the bubble is at the high end, close to where the pipe comes down from the sink. :)
