We moved into a house and one shower has handles for both hot and cold. There is no hard off and on. You can spin the handles indefinitely and it will cycle from on to off. I dislike this because you have to get it just right to be off. Will just replacing the handle inserts fix this or will this require replacing the whole assembly?

What make faucet do you have? It may just be the handle or it could be the cartridge as Liquid said.

yes, to do this you must turn the water off.
see the silver centers of the handles, there will likely be a way to free them and expose a nut underneath, try to find the edge of the silver center and guntly pry with a flathead. if so then try tightening the nut in the middle.
that there is no definite stop I think a part is either broken internally, or was not installed originally but there are tons of makes for these, some designed better than others
