I have an American Standard toilet purchased in 2004. It has a weird cylinder/tube valve mechanism that gets stuck open when flushed. The other toilets in my house have the standard flapper. I have had no luck finding anything remotely close to this mechanism on-line or in the stores. I have tried to find the toilet model number under the lid and inside the tank with no luck.

Any ideas what to try next? I have thought about putting a flapper in but it looks like the whole is a different size.

You have a American Standard Champion 4 toilet, The champion 4 old style flush valve is one of the worst flush valve AMSTD ever made. As John stated American Standard do have a upgrade flush valve for that toilet.
 
the gasket is concaved you can purchase a new one at dixieline the gasket is what is making the plunger hard to flush. It will eventually break the plastic flush lever
